The following is a list of important items you will need to bring with you to registration.
All the forms you will need are printable from our schedule & forms tab.
Please bring all the completed forms list below with a check to registration.
Feel free to " Contact Us " from this website with any questions.
- Completed Parent Code of Conduct form. ( please print a copy for your records )
- Completed Informed Consent form.
- Completed Participant Application form. Heights & weights will be taken at registration.
- Completed Volunteer Sign-Up sheet.
- Your player/cheerleader accompanied by at least one parent or legal guardian.
- Registration fee, cash or check. $150 per player, second player in the same family is $100, third and additional players $75.00 each. Cheerleading registration fee is $50 per participant.
